Waivers
All these PDF waiver forms are linked on the main SCA Corporate Website, in the Library of SCA Documents. Each will open in a separate window.
Adult Waivers
- Per Society rules - If a person (minor or adult) can present a blue SCA membership card, this indicates that there is a signed liabity waiver on record with SCA Corporate. You will only have to sign any SPECIAL waivers that might be required by some site owners or equestrian waivers required by the event.
- If you forget to bring your membership card or present a white card, you will be required to sign the liability waiver on site.
- In order to pre-register for the event or as a merchant, you will be required to mail in a completed waiver with your registration paperwork.
- Download the Adult Waiver
Minor Waivers
A responsible adult must accompany all minors (17 years or under in Calif.), who are not legally emancipated, to the war site. This must be a parent or court-appointed guardian or the adult designated on the appropriate waivers signed and notarized by the parent or legal guardian.
For more details see the Minor's Policy
Parental Consent Forms - In Caid, two forms are used for parents who allow their minors to attend an event without them.
- A Minor's Consent to Participate and Hold Harmless Agreement Waiver (PDF available from SCA.org) is used to show a parent's consent.
- A Medical Authorization for Minors with space for notorization (PDF available from SCA.org) is used to designate temporary guardianship; it allows medical treatment to be provided in the event of an emergency.
- Both forms must be signed by the parent and notarized.
